Carers' Emergency Back-Up Plans
This council-funded scheme replaces Carers’ Alert Cards. It aims to give some peace of mind for main carers that their cared-for person(s) will be attended to, should they be unexpectedly absent due to an emergency.
The carer puts in the plan their preferred relatives, friends or current care services who are prepared to step in (or are just keyholders). Further home-based back-up services may be provided free by the council for up to 48 hours (72 at weekends), allowing time for ongoing support to be arranged.

Care Passports
Look out for our Care Passports in all the local hospitals and residential care settings. The bright yellow passports take the form of a notebook in which carers can note the special needs of those they care for and can write down questions they wish to ask nurses or consultants. The passport is then kept with the patient and referred to by hospital staff. You can get one from us, which you can use every time the person you care for is admitted to residential care or hospital, and remind the Ward Manager and nursing staff to refer to it, because it is a useful tool for them as well!
