Making a referral
If you are a carer you can contact us directly and speak to anyone in the team. We are here to help you and will take your details over the phone.
If you are a health professional, please read the information below about how to go about making a referral to The Carers Centre.
If you know of an adult carer aged 18 or over, first let the carer know you are making a referral and then complete as much information on the referral form as possible. We will contact the carer to outline the support we can offer. This could include information over the telephone call with some follow up support or time-limited one-to-one casework offering emotional support, information and advocacy. We also make referrals for other services such as counselling, money advice or housing advice.

If a potential young carer is identified please in the first instance refer the young person for exploration and support via.

Following the Assessment stage, if a caring role is identified, the Front Door for Families team can refer the young person to our young carers’ team.
Our team offers a range of groups and activities for young carers throughout the year. Some are fun breaks and new experiences, whilst others are more focused on supporting young carers around specific needs and issues. Support Workers also provide a casework service. Depending on the level of need of the young carer this can include the following: 1:1 support sessions; inputting to Early Help/Child In Need/Safeguarding plans and meetings; and advocating with services on the young carer’s behalf.
